Please read through the following information about this service before you decide whether or not to register and receive condoms via this postal service.

What is the purpose of this service?

The purpose of this service is to provide individuals over the age of 17 with condoms in Ireland. Condoms give the best protection against Sexually Transmitted Infections and are 98% effective against pregnancy when you use them correctly and every time you have sex. The condoms provided are NOT Latex Free.

What information will you ask me?

We will ask you for your age, full name, address, e-mail address and contact telephone number so that you can register with the online service. By completing this form and providing the information you are consenting that the Sexual Health Centre retain your details and use them to facilitate this service in line with our Data Protection Policy. Once registered, you will be redirected to a page which will include some sexual health information, a condom demonstration video and a link to our data protection, complaints and children and vulnerable adult policies.

Once we’ve received your information, we will call you to confirm your details, talk to you about your sexual health and wellbeing and answer any questions you may have. Once your details have been confirmed we will add your details to our database. This means we will not need to call you again when ordering in future, as we will have documented your confirmation. If you cannot be reached on this number, we will not process your order.

Who is eligible for this service?

This service is for those aged 17 years and older. Remember in Ireland the legal age of consent is 17 years and so the service will only deliver to those of 17 years or older. This is why we are asking you to confirm your age. Any concerns will be dealt with in line with our Children and Vulnerable Adult Policy.

How will I receive the condoms?

You should receive the condoms within 5 working days of your order and confirmation. The condoms will be in discrete, plain packaging. The pack will include sexual health information and advice as well as contact information for the Sexual Health Centre.

What will happen to my information? · Your data will be kept in accordance with the Sexual Health Centre’s Data Protection Policy.

· Your data will be used to a) make contact with you in relation to your order and b) process your order with the Sexual Health Centre.

· The Sexual Health Centre team will have access to your data.
